Medical Insurance

As of this time, I am contracted with Landmark Insurance company, Premier Provider Network, Inc. (PPNI), and OptumHealth which includes: United Healthcare, PacifiCare Health Systems, Medical Resource, LLC, and Spring Hill School. For patients covered by other insurance providers, I am able to assist with insurance billing if your plan covers acupuncture and will accept an out-of-network provider (usually this occurs with a P.P.O plan). Contact your medical insurance company to see if acupuncture is a covered service under your plan and if you are able to see providers out-of-network. Ask for specific details of coverage since these vary for each policy. Specifically ask about deductibles, diagnosis exclusion, number of visits allowed, and if there is a maximum amount paid for each visit. Patients are responsible for the portion of the fee not covered by the insurance company. Payment for acupuncture appointments may also be used toward flexible spending accounts- talk to your employer about this program.

Cancellation Policy

Patients are asked to make any schedule changes or cancellations prior to 24 hours of their scheduled time. Patients will be charged the full service fee for cancellations made within 24 hours of the scheduled appointment. Insurance does not cover missed appointments, so insurance patients will be charged in full.
